What should I bring to my appointment?

• Your insurance card and a photo ID
• Completed new patient forms (new patients)
• A list of your current medications and allergies
• Any diagnostic films, imaging, or test results ordered or performed by another physician
• Your co-payment, if your plan requires one

Insurance and payment

Women’s Surgical Specialists accepts Medicare and most major insurance plans. Please contact your insurance company, or call our office at (954) 771-8888, to find out whether we accept your plan.

If your plan requires a co-payment, co-insurance or deductible, payment is due at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, Discover and American Express.

Billing questions

For questions about a bill or your insurance, call the billing department at (954) 771-8888.

Prescription refills

Prescriptions and refills for established patients are issued during regular office hours, Monday–Friday, 9:00 am–4:30 pm. Call (954) 771-8888 to request a refill.

No refills are issued during evenings, weekends or holidays, because the doctors do not have access to your medical records at those times. Please plan ahead so your medication is not interrupted.

After hours and emergencies

For an urgent medical need after regular hours, call (954) 771-8888. A doctor is on call 24 hours a day, and your call will be returned promptly.
